It looks as if large canals are not difficult to clean but actually the opposite is true. All resources have to be utilized in order to thoroughly clean the canal wall all around well. Important resources are:
EDTA (to remove the smear layer from the canal wall)
Hydrogen peroxide (in combination with hypochlorite it certainly has a foaming effect)
Filling large canals is also more difficult. The easiest method is with System B and Obtura II . With System B a guttapercha main point is melted off until approximately 5 mm from the apex and then condensed vertically. Now the needle of Obtura II is kept on the condensed gutta-percha. After a few seconds the surface gets plastic. By applying low pressure on the handle of the Obtura pistol the liquid gutta-percha is now sprayed into the canal. Because of the counterpressure of the gutta-percha, the needle is, so to speak, pressed out of the canal. After the canal has been filled the gutta-percha is vertically condensed until it cools off.
Needless to say, there are also other methods to fill large canals, such as:
Lateral condensation (long-lasting additional positioning of gutta-percha points)
Exclusively Obtura II (the apical blocking is less predictable)
Exclusively System B (the backfill is more difficult and therefore, it takes more time)
